Olympiacos Soccer Club Chicago does not provide health or accident insurance coverage for participants. Registration assumes responsibility on the part of the registrant and/or parent or legal guardian, subject to the terms and limitations set forth below.

1. As a participant or the parent/legal guardian of a participant in any Olympiacos Soccer Club Chicago camp, tryout, training session, practice, game, tournament, clinic, program, event or other club activity, I recognize and acknowledge that participation in soccer and related athletic activities involves inherent and other risks, including but not limited to running, jumping, kicking, physical contact, collisions with players or objects, falls, weather conditions, playing surfaces, equipment, transportation where applicable, and other risks that may result in bodily injury, illness, disability, property damage or, in rare circumstances, death. I knowingly and voluntarily ASSUME such risks to the fullest extent permitted by law.
2. For myself individually, and only to the extent legally permitted with respect to my ward, I WAIVE, RELEASE AND DISCHARGE Olympiacos Soccer Club Chicago and its officers, directors, managers, trainers, coaches, volunteers, agents, representatives, employees and program board members from claims arising from ordinary negligence associated with participation in club activities, facilities or programs, to the fullest extent permitted by applicable law. This provision is not intended to waive any right or claim that cannot legally be waived.
3. To the fullest extent permitted by law, I agree to indemnify and hold harmless Olympiacos Soccer Club Chicago and its officers, directors, managers, trainers, coaches, volunteers, agents, representatives, employees and program board members from claims, expenses or losses arising from my own acts, omissions or violations of club rules in connection with participation in club activities.
4. In the event of an emergency, I authorize club officials to obtain from any licensed hospital, physician, emergency medical provider and/or other qualified medical personnel any treatment reasonably believed necessary for me or my ward’s immediate care. I understand that Olympiacos Soccer Club Chicago is not responsible for the cost of such treatment and that I am responsible for any medical expenses incurred.

As part of player development, evaluation, safety, team administration and performance tracking, Olympiacos Soccer Club Chicago may collect and maintain certain physical, athletic and developmental information regarding participants.

1. AUTHORIZATION TO COLLECT PLAYER DATA.
I authorize Olympiacos Soccer Club Chicago to collect, record, store, use and analyze information relating to my or my child’s participation and athletic development (“Player Data”). Player Data may include, but is not limited to, height, weight, age, playing position, attendance, training participation, coach evaluations, soccer skill assessments, sprint times, speed, agility, endurance, strength, flexibility, jumping ability, reaction time, workload and other similar physical, developmental or athletic-performance measurements.

2. PURPOSE OF COLLECTION.
I understand and agree that Player Data may be used for legitimate soccer and club-related purposes, including player development, performance tracking, individualized training, coaching decisions, player evaluations, team placement, training-load management, safety, program improvement, talent identification, internal statistical analysis and communication with the participant and his or her parent/legal guardian.

3. ACCESS, STORAGE & AUTHORIZED SERVICE PROVIDERS.
I authorize the Club to maintain Player Data in paper or electronic form and through Club-authorized software platforms, databases, cloud services, athletic-performance systems and other technology providers. Player Data may be made available to Club directors, administrators, coaches, trainers, evaluators and authorized contractors or service providers when reasonably necessary to perform Club-related functions. Such information will not knowingly be sold as consumer data by Olympiacos Soccer Club Chicago.

4. SECURITY & ELECTRONIC DATA RISK.
Olympiacos Soccer Club Chicago will use reasonable administrative, technical and organizational measures appropriate to the nature of the information to protect Player Data. I understand, however, that no paper record, computer system, cloud service, mobile application, email system, database or electronic transmission can be guaranteed to be completely secure or free from unauthorized access, technical failure, cyberattack or other security incident.

To the fullest extent permitted by law, I acknowledge and assume the ordinary risks inherent in the authorized electronic storage, processing and transmission of Player Data and release the Club from claims arising solely from the Club’s good-faith collection, use or storage of Player Data consistent with this authorization. Nothing in this Agreement releases or excuses conduct that cannot legally be waived, including liability arising from intentional wrongdoing, willful or wanton misconduct, gross negligence where applicable, or a violation of an applicable privacy or data-protection law.

5. HEALTH INFORMATION & HIPAA.
I understand that the collection of athletic measurements or performance information by a youth soccer organization does not, by itself, make Olympiacos Soccer Club Chicago a health-care provider or an entity subject to the Health Insurance Portability and Accountability Act (“HIPAA”). The Club does not provide medical diagnosis or medical treatment through the collection of Player Data. If particular information or a particular relationship is subject to HIPAA or another applicable health-privacy law, nothing in this Agreement is intended to waive rights or obligations imposed by that law.

Information voluntarily provided to the Club concerning injuries, allergies, medical restrictions, medications, emergency conditions or other health-related matters may be used and shared internally with appropriate coaches, administrators, trainers or emergency personnel when reasonably necessary for the participant’s health and safety and otherwise handled in accordance with applicable law.

6. CHILDREN & ONLINE INFORMATION.
If the participant is under 13 years of age, I represent that I am the participant’s parent or legal guardian and authorize the collection and use of Player Data through Club-authorized systems for the purposes described above. Where additional notice, parental verification, consent or privacy procedures are required by applicable children’s online privacy laws, the Club may provide a separate privacy notice or consent process. Nothing in this Agreement is intended to eliminate any requirement imposed by applicable law.

7. BIOMETRIC TECHNOLOGY.
This authorization for ordinary player measurements and performance metrics is not intended to authorize the collection of fingerprints, retina or iris scans, voiceprints, scans of hand or face geometry, or similar biometric identifiers used to identify an individual. If Olympiacos Soccer Club Chicago later implements technology involving biometric identifiers or biometric information regulated by applicable law, any additional notice, written consent, retention policy or other legally required procedure will be addressed separately.

8. RETENTION & REQUESTS.
Player Data may be retained for as long as reasonably necessary for the purposes described in this Agreement, for legitimate Club administrative or historical purposes, or as otherwise required or permitted by law. A parent/legal guardian may contact Olympiacos Soccer Club Chicago regarding material inaccuracies in Player Data or to request that information no longer reasonably needed by the Club be deleted, subject to legal, safety, recordkeeping and legitimate operational requirements.

9. ACKNOWLEDGMENT & CONSENT.
By signing or electronically accepting this Agreement, I acknowledge that I have been informed that Olympiacos Soccer Club Chicago may collect and use the Player Data described above. I knowingly and voluntarily consent to such collection, use, storage and processing for the purposes stated in this Agreement. This consent does not waive any right that cannot legally be waived under federal, state or local law.

I give Olympiacos Soccer Club Chicago permission to photograph, videotape and otherwise record the participant during Club programs, games, practices, tournaments and events and to use the participant’s name, image, likeness, voice, photograph and video for legitimate Club promotional, informational and educational purposes, including Club websites, social media, newsletters, advertisements, printed materials, broadcasts and other media, without compensation.

I also authorize Olympiacos Soccer Club Chicago to use parent/legal guardian and participant contact information, including email addresses and telephone numbers where provided, for Club-related communications, registration information, schedules, program updates, newsletters and other legitimate Club purposes.

In consideration of being allowed to participate in any way in the program, related events and activities, I acknowledge, appreciate and agree that:

1. I am aware that participation may involve a risk of direct or indirect exposure to communicable diseases, including but not limited to influenza, COVID-19, respiratory illnesses and other infectious or communicable diseases and any mutations or variations thereof.
2. For myself, my heirs, assigns, personal representatives and next of kin, and only to the extent legally permitted with respect to my ward, I HEREBY RELEASE, INDEMNIFY AND HOLD HARMLESS Olympiacos Soccer Club Chicago, its officers, officials, agents, employees, coaches, volunteers, other participants, sponsors, advertisers and, where applicable, owners and lessors of premises used to conduct Club activities (“RELEASEES”) from claims arising from ordinary risks associated with exposure to communicable disease during participation, including ordinary negligence to the extent such claims may legally be released. Nothing herein is intended to release claims or rights that cannot legally be waived.
